29 CFR 1926.453(b)(2)(iv): Employee(s) were not standing firmly on the floor of the basket, were sitting or climbing on the edge of the basket, or were using planks, ladders, or other devices for a work position:  a. On or about 08/14/2019, building F - an employee climbed over the basket aerial lift to enter an open bottom catwalk like section to conduct framing work, exposing the employees to about 18 feet fall hazard.

29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:  a. On or about 08/14/2019, building F - personal fall protection was not attached to the basket anchor point and one employee was not using a harness while conducting framing work from an aerial lift, exposing the employees to about 18 feet and 16 feet fall hazard.

29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes:   a. On or about 08/14/2019, building F - an employee was exposed to about 18 feet fall hazard while conducting framing work from inside an area missing the bottom section or flooring, in that, fall protection was not used.
