29 CFR 1910.132(d)(2): The employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done:  Left Bank, LLC DBA Dixie Elixir at 4990 Oakland Ave, Denver, CO 80239: On or before 8/14/2019, the employer did not verify, through a written certification, the identity of the workplace evaluated, the person certifying that the evaluation had been performed, and the date the hazard assessment was done. The employer has implemented work rules for the use of PPE; however a workplace assessment has not been performed and certified by the employer. Employees were exposed to physical and chemical hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling:  Left Bank, LLC DBA Dixie Elixir at 4990 Oakland Ave, Denver, CO 80239: On or before 8/14/2019 the employer did not ensure that metal electrical outlet boxes were used in accordance with listed instructions. A flexible cord was suspended from the ceiling attached to a metal outlet box with pre-punched knockouts. This exposed employees to an electrical shock hazard.
