The employer has implemented work rules for the use of PPE; however a workplace assessment has not been performed and certified by the employer. Employees were exposed to physical and chemical hazards.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: Left Bank, LLC DBA Dixie Elixir at 4990 Oakland Ave, Denver, CO 80239: On or before 8/14/2019 the employer did not ensure that metal electrical outlet boxes were used in accordance with listed instructions. A flexible cord was suspended from the ceiling attached to a metal outlet box with pre-punched knockouts. This exposed employees to an electrical shock hazard.
