29 CFR 1926.453(b)(2)(iv): Employee(s) working in an aerial lift were not standing firmly on the floor of the basket. On or about August 15, 2019 an employee of Thornco, LLC was observed standing on the rail of the basket of a Genie lift. Employees were installing building material to the ceiling of the canopy on the south side of the building.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer. On or about August 15, 2019 employees were not trained in fall protect and there was a written certification record for each employee that might be exposed to a fall hazards. Employee(s) were observed in the basket of an aerial lift approximately 8 ft. from the ground.
