29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a) On or about 8/9/19 at a roof replacement project located at 35 Abbottsford Place, Buffalo, New York; employees performing roofing activities from a ladder jack scaffold approximately 24 feet high were not protected from falling by a personal fall arrest system.  NO ABATEMENT CERTIFICATION REQUIRED  United Roofing & Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1236067, citation number 1, item number 1 and was affirmed as a final order on 1/15/18, with respect to a workplace located at 280 Oehman Boulevard, Cheektowaga, New York.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) On or about 8/9/19 at a roof replacement project located at 35 Abbottsford Place, Buffalo, New York; employees working on the roof were removing shingles on the roof of a two story house. The roof had a 12 in 12 pitch and an eave heights of approximately 26 feet. There was no fall protection in use.  NO ABATEMENT CERTIFICATION REQUIRED  United Roofing & Siding was previously cited for a violation of this occupational safety and health standard or its equivalent standard, which was contained in OSHA inspection number 1236067, citation number 1, item number 2 and was affirmed as a final order on 1/15/18, with respect to a workplace located at 280 Oehman Boulevard, Cheektowaga, New York.
