29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval: a) On or about August 13, 2019, warehouse receiving area, employee(s) were working from a 48"x 72" man basket that sat on top of a 40" x 48" wooden pallet that was not secured to the Crown high lift, serial #1A319595. The employee was changing an AC filter at a height of approximately 25 feet.
