29 CFR 1926.56(a): While work was in progress, construction areas, ramps, runways, corridors, offices, shops or storage areas were not lighted to the minimum illumination intensities listed in table D-3 of this subpart:      (a)  Industrial Vacuum Service Inc, at 4550 Kingston Street, Denver, CO: On and preceding 8/13/19, while work was in progress, construction areas, ramps, runways, corridors, offices, shops or storage areas were not lighted to the minimum illumination intensities listed in table D-3 of this subpart.  Employees of Industrial Vacuum Service Inc removed existing roofing materials on the low slope roof.  Workers performed these operations during nighttime.  The employer utilized a warning line system that partially enclosed the work area.   Warning lines were not erected from the ladder access point to the area where roof material removal was taking place.  Workers traveled between the active work area and the ladder.  The travel path was not illuminated.  This condition exposed employees to tripping hazards and an approximate thirty-two foot fall hazard.

Employees removed existing roofing materials on the low slope roof.  Employees performed these operations in proximity to the roof edge.  The employer utilized a warning line system that partially enclosed the work area.  The partial warning line was erected less than six feet from the roof edge.  This condition exposed employees to an approximate thirty-two foot fall hazard.

Employees removed existing roofing materials on the low slope roof.  The employer used a Cleasby Hurricane gasoline powered scraper in addition to hand removal of roofing. The partial warning line was erected less than six feet from the roof edge.  This condition exposed employees to an approximate thirty-two foot fall hazard.

29 CFR 1926.502(f)(1)(iii): Points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines:  (a)  Industrial Vacuum Service Inc, at 4550 Kingston Street, Denver, CO: On and preceding 8/19/19, points of access, material handling areas, storage areas, and/or hoisting areas were not connected to the work area by an access path formed by two warning lines.  Employees removed existing roofing materials on the low slope roof.  Work was taking place in the northeast corner of the roof.  Workers accessed the roof via a fiberglass extension ladder on the south side of the building.  Industrial Vacuum Service Inc utilized a warning line system that partially enclosed the work area.  Warning lines were not erected from the point of access several hundred feet to the area where workers were removing roofing.  This condition exposed employees to an approximate thirty-two foot fall hazard.

Employees used gasoline to fuel the Cleasby Hurricane powered scraper used to remove roofing and to fuel the ATV used on the roof.  The employer did not ensure that its written hazard communication program as implemented included the methods used to provide on-site access to SDSs for each hazardous chemical brought to the site by Industrial Vacuum Service Inc that other employers' employees may be exposed to while working at the multi-employer worksite.  Hazardous chemicals brought to the work site by the employer included, but were not limited to the following:  1.  Gasoline.

29 CFR 1926.852(a): Material was dropped to points lying outside the exterior walls of the structure which had not been effectively protected:  (a)  Industrial Vacuum Service Inc, at 4550 Kingston Street, Denver, CO: On and preceding 8/13/19, material was dropped to points lying outside the exterior walls of the structure which had not been effectively protected.  Employees removed existing roofing materials on the low slope roof.  Employees dropped waste materials into a dumpster outside the north exterior wall of the building approximately 32 feet below the roof edge.  Neither a chute directed debris into the dumpster nor was a barricade erected around the dumpster.  Large pieces of debris, up to approximately four feet, were dropped from the roof edge.  The debris was distributed on the ground around the dumpster.
