(a) Vernon Nagel, Inc. on or about August 19th, 2019, at the location of 906 Loch Haven Blvd., Maumee, OH, 43537: An employee was riding in the bucket of a Komtasu WA320-8 wheel loader and using it as a temporary work platform while trimming tree branches with a chainsaw.  The employee was exposed to fall hazards of approximately 10 feet while standing in the bucket without any protection from falls.  The employee was exposed to potential crushing hazards from the wheel loader while entering, exiting, and riding in the bucket.
