29 CFR 1910.217(b)(4)(i): Pedal mechanism(s) on mechanical power press(s) full revolution clutches were not protected to prevent unintended operation from falling or moving objects or by accidental stepping onto the pedals: On or about 8/22/2019, Machine #25, ROUSSELLE STAMPING PRESS, had a foot pedal that was not protected from falling or moving objects or by accidental stepping onto the pedal which could actuate the press to cycle unexpectedly.

29 CFR 1910.217(c)(2)(i)(b): Point of operation guard(s) on mechanical power press(s) did not conform to the maximum permissible opening(s) specified in Table O-10: On or about 8/22/2019, Machine #25, ROUSSELLE STAMPING PRESS, Guard on the front of the press had approximately a 1 1/2" opening under the guard which did not meet the maximum allowable opening of Table O-10 creating an amputation hazard for the operator. Openings on the side of the press and back of the press also exceeded the allowable opening size.
