29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident: In the kitchen at 587 College Highway, Southwick, MA, on and before August 19, 2019, the circuit breaker panel was not labeled to indicate the purpose of each breaker exposing employees to electrical shock hazards while turning the kitchen lights on and off each day from the panel.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i): In the kitchen at 587 College Highway, Southwick, MA, on and before August 19, 2019, employees working in the kitchen are exposed to electrical shock hazards from a blank circuit breaker without a filler plate in an electrical panel. Employees turn the kitchen lights on and off each day from the electrical panel which has an exposed, live busbar.
