29 CFR 1926.20(b)(1): It shall be the responsibility of the employer to initiate and maintain such programs as may be necessary to comply with this part.    On or about 21 August 2019, at the job site located at 38 Gilbert Ave. Clarendon Hills Il, 60514, the employer failed to develop and maintain safety programs to protect the employees from electrical and fall hazards.  Employees were exposed to electrical hazards when using equipment not protected by a ground fault circuit interrupter (GFCI).  Additionally, employees were exposed to fall hazards while working at heights.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

29 CFR 1926.20(b)(2): Such programs shall provide for frequent and regular inspections of the job sites, materials, and equipment to be made by competent persons designated by the employers.    On or about 21 August 2019, at the job site located at 38 Gilbert Ave. Clarendon Hills IL, 60514, the employer did not conduct frequent and regular inspections of job sites, materials, and equipment by a competent person.  Employees were exposed to electrical hazards when using equipment not protected by a ground fault circuit interrupter (GFCI).  Additionally, employees were exposed to fall hazards while working at heights.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

Employees were exposed to electrical shock hazards when using equipment not protected by a ground fault circuit interrupter (GFCI). 29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.    On or about 21 August 2019, at a worksite located at 38 Gilbert Ave, Clarendon Hills, IL. 60514, employees were not trained to recognize and minimize fall hazards prior to conducting sheeting activities on the second level of the residential structure. This exposed employees to falls of approximately 12 feet to the concrete below.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Several cords had damage to their outer protective layers, exposing employees to electrical shock hazards.     Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).

Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.     On or about 19 August 2019, at the job site located at 38 Gilbert Ave. Clarendon Hills Il, 60514, the employer failed to protect the employees from a fall hazard when they were working above 6 feet. The employees were exposed to a 12 feet 5 inch fall hazard without having any sort of fall protection devices, railings or systems in place.    Abatement documentation is required for this item in accordance with 29 CFR 1903.19(d).
