29 CFR 1926.453(b)(2)(v): A body belt with lanyard attached to the boom or basket was not worn by employee(s) working from an aerial lift: (a) On or about August 17, 2019, the employer did not ensure that employees operating an aerial lift used a body belt or a body harness with a lanyard attached to the boom or basket. Employees of RV Tile and Cabinetry, LLC were exposed to falls of up to 14 feet while installing tile siding on a commercial building roof while on a Genie S-45 aerial lift. The employees did not wear lanyards attached to the boom or basket of the aerial lift. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").

29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: (a) On or about June 29, 2019, the employer did not ensure that aerial lift operators were trained by a qualified person to recognize, control, and minimize the hazards associated with aerial lifts. Employees of RV Tile and Cabinetry,LLC. were exposed to falls of up to 14 feet while operating a Genie S-45 aerial lift. The employees were not trained to operate the aerial lift. Abatement Note: Abatement certification is required for this item (see enclosed "Certification of Corrective Action Worksheet").
