Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Northeast side of roof, on or about 8-20-19: Six employees were working on a sloped roof (4:12 pitch) and were not protected from falls, exposing the employees to a fall hazard of 17 feet 10 inches to the pavement below.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: a) North side of parking area, on or about 8-20-19: Six employees were using a portable fiberglass extension ladder for roof access/egress, with side rails that were 18 inches above the landing surface, exposing the employees to a fall hazard of 17 feet 10 inches to the pavement below.
