29 CFR 1926.20(b)(2):  The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:       (a) Vivint Solar Developer LLC dba Vivint Solar at 1019 Cable St., Brighton, CO 80603:  On or before July 30, 2019 the employer did not ensure a safety program was developed and implemented for the work environment, to include regular inspections made by a competent person of the job-site, materials, and equipment.  On July 29, 2019 three employees were working on the roof of a two-story residential home, when one employee fell off the roof and sustained serious injuries.  The employee did not utilize a fall protection system.  This condition exposed employees to fall hazards and injuries resulting from a fall off a roof.
