29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     On or about 08/14/2019, Sams Club, located at 2100 Maplewood Commons Drive, Maplewood, MO, the victim was exposed to hazards related to the Hobart meat cutter band saw (Model: 6801, S/N: 12Y80963).  The guarding to protect body parts from the saws blade was not properly adjusted to prevent accidental contact with the saws blade.  This resulted in an amputation.  The training video did address the blade guard but did not provide the required adjustment height for the blade guard.
