29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: The employer is failing ensure that employees are protected from machine moving parts hazards. This most recently occurred at 4657 G Street, Omaha, Nebraska 68117 on August 17, 2019. The employer failed to ensure that machine guarding was used on the Laminator 2. An employee reached into an opening on the laminator when the moving conveyor came in contact with the employee's arm causing a fractured left hand.
