29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following:  Guardrail systems, safety net systems, or personal fall arrest systems:  a) 3914 Hamilton Blvd, Theodore, AL (Shop and Yard areas): On August 21, 2019 and at times prior, the employer exposed employees to fall hazards of up to 11 feet 9 inches in that employees were allowed to access tops of vacuum trucks and when accessing the viewing portals without the use of fall protection.
