29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift:   a) On or about 08/21/2019 at 1250 Honore Ave. Sarasota, FL, an employee working from a United Rental S-65 aerial lift was exposed to a 22 ft. fall hazard, in that, a fall protection system was not used while installing plywood panels on the wall of the residential apartment building.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) On or about 08/21/2019 at 1250 Honore Ave. Sarasota, FL, employees were exposed to a 32 ft. fall with a 5/12 slope hazard, in that, a fall protection system was not used while installing plywood panels on the roof of a residential apartment building.      Esperanza Quality Construction Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1385730, citation number 1, item number 1 and was affirmed as a final order on October 11, 2019, with respect to a workplace located at 1099 Montgomery Bell in Wesley Chapel, FL.  Esperanza Quality Construction Corp was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1375228, citation number 1, item number 1 and was affirmed as a final order on March 12, 2019, with respect to a workplace located at 1059 Montgomery Bell Road in Wesley Chapel, FL.
