29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: a) On or about 08/21/2019, at the Commingle Area, the MCC-3 460-VAC electrical cabinet was bypassed to feed the M-66 conveyor line and the door of the cabinet could not effectively close. Employees accessed the cabinet to actuate a switch that controlled the M-66 conveyor, exposing to electrical shock hazards form live parts inside the MCC cabinet and fire hazard from dust that entered the open cabinet.
