29 CFR 1910.22(d)(2): Hazardous conditions on walking-working surfaces were not corrected or repaired before an employee uses the walking-working surface again.  Where the correction or repair could not be made immediately, the hazard was not guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired:   a) Romano L. Mazzoli Federal Building located at 600 Martin Luther King Jr. Place, Louisville, Kentucky - East Stairwell:  On or about August 21, 2019, workers were permitted to continue using the stairwell and the handrails were not repaired.       b) Romano L. Mazzoli Federal Building located at 600 Martin Luther King Jr. Place, Louisville, Kentucky - Center Stairwell:  On  or about August 21, 2019, On or about August 21, 2019, workers were permitted to continue using the stairwell and the handrails were not repaired.     c) Romano L. Mazzoli Federal Building located at 600 Martin Luther King Jr. Place, Louisville, Kentucky - West Stairwell:  On or about August 21, 2019, On or about August 21, 2019, workers were permitted to continue using the stairwell and the handrails were not repaired.

Handrails and stair rail systems were not smooth-surfaced to protect employees from injury, such as punctures or lacerations, and to prevent catching or snagging of clothing:   a) Romano L. Mazzoli Federal Building located at 600 Martin Luther King Jr. Place, Kentucky - East Stairwell:  On or about August 21, 2019, the handrails on the stairs leading from the 1st to the 10th floors had several hazards in numerous locations including rails that were separating crating pinch points and protruding screws, nails, bolts, nuts and sharp objects.  b) ) Romano L. Mazzoli Federal Building located at 600 Martin Luther King Jr. Place, Kentucky - Center Stairwell:  On or about August 21, 2019, the handrails on the stairs leading from the 1st to the 10th floors had several hazards in numerous locations including rails that were separating crating pinch points and protruding screws, nails, bolts, nuts and sharp objects.  c) ) Romano L. Mazzoli Federal Building located at 600 Martin Luther King Jr. Place, Kentucky - West Stairwell:  On or about August 21, 2019, the handrails on the stairs leading from the 1st to the 10th floors had several hazards in numerous locations including rails that were separating crating pinch points and protruding screws, nails, bolts, nuts and sharp objects.

Handrails and the top rails of stair rail systems are capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in any downward or outward direction within 2 inches (5 cm) of any point along the top edge of the rail.  a) Romano L. Mazzoli Federal Building located at 600 Martin Luther King Jr. Place, Kentucky - East Stairwell:  On or about August 21, 2019, the handrails on the stairs leading from the 1st to the 10th floors had guardrails that were not secured or separating from the wall and broke.
