29 CFR 1926.102(a)(2): The employer shall ensure that each affected employee uses eye protection that provides side protection when there is a hazard from flying objects. Detachable side protectors (e.g. clip-on or slide-on side shields) meeting the pertinent requirements of this section are acceptable. On or about August 21, 2019, at the previously addressed job site, employees using pneumatic power tools while roofing did not use eye protection exposing the employees to flying objects to the eyes and eye injuries.

On or about August 21, 2019, at the previously addressed job site, the portable ladder(s) used to access the roof did not extend at least three (3') feet above the upper landing surface, thereby exposing employees to the hazards associated with falls.
