29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.. (a) On or about August 19, 2019, an employee was observed walking on the main top roof of the Hyatt Regency at 333 W. Kilbourn Ave., Milwaukee, WI 53203. The employee was walking around and in between uncovered/unprotected skylights while adjusting wires, exposing the employee to a fall hazard of greater than 170 feet to the interior lobby floor.
