29 CFR 1910.22(a) (2): Floor(s) of workroom(s) were not maintained in a clean and, so far as possible, a dry condition: a. 1445 Mineral Springs Road, Elberton, GA (Production Area) - Employees were exposed to slip hazard. Employees in the production area are exposed to slippery floors. The machines leaks oil, which the employer tries to clean up by using the same dirty, oily water to clean up oil from the floor, in other production areas. The floors are very slippery, causing employees to slip and slide on the floor, in close proximity to the machines, Employees are not required to wear slip-resistant shoes, on or about August 21, 2019.

29 CFR 1910.22(c): 29 CFR 1910.22(c): The employer did not ensure that each employee used a safe means of access and egress to and from walking-working surfaces: a. 1445 Mineral Spring Road, Elberton, GA (Outside Shed Area) - Employees are exposed to trip, fall, and bumping into material hazard. Employees on 2nd and 3rd shift are required to go outside and retrieve empty dunnage/totes from the outside coverage shed and bring back into the warehouse area. The outside shed is not equipped with lights for employees to see the surroundings. The walking-working surfaces area is very dark and is not illuminated, the outside shed is not provided with a safe means of access and egress to and from the shed, on or about August 21, 2019.
