29 CFR 1910.151(c): Where the eyes or body of any person may be exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body shall be provided within the work area for immediate emergency use. a) Warehouse, Battery Charging Station: On or about August 21, 2019, and at times prior, the employer exposed employees to eye injuries, in that frequent inspections of an Showerhead Eyewash Station (Bradley Showerhead Model S19314TT) were not performed to ensure it was in a suitable condition for immediate use.

29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition. a) Warehouse: On or about August 21, 2019, and at times prior, the employer exposed employees to struck-by hazards in that Forklift (Crown Electric Forklift Truck Model RC3020 Serial Number 1A188702) had a frayed hydraulic hose located at the mast but was not taken out of service.
