29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system.  (a) Employees conducting roofing work on an 8:12 pitch roof were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately Twelve (12) feet on the west side of the structure. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  (a) Employee(s) laying shingles as part of roofing work and standing on the top of the 8/12 pitched roof structure, had not received training that would allow the employee(s) to recognize the related hazards and to properly utilize the proper fall protection needed to minimize the hazard. 29 CFR 1926.1053(b)(1): Where a portable ladder(s) was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    (a) The portable Werner extension ladder erected on the South side of the structure, and being used for access to the roof, did not extend at least three (3) feet above the edge, and/or was not secured to the structure when it was not sufficiently extended. (a) The employer did not insure that only approved safety cans were used for the handling and use of flammable liquids when dispensed in quantities of 5 gallons or less. Plastic five (5)  gallon gas cans were used to store and dispense gasoline to the portable gasoline powered air compressor.
