29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: (a) Slab Saw Station - On or about August 19, 2019 employees were exposed to amputation hazards, in that employees were allowed to perform jam clearing operations on equipment such as, but not limited to, Lincoln Machine Automatic Slab Saw Serial # 89170-D, without utilizing energy control procedures to prevent accidental startup of the machine.

29 CFR 1910.147(c)(7)(i)(A): Each authorized employee had not received training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control: (a) Slab Saw Station - On or about August 19, 2019 the employees were exposed to amputation hazards, in that they were allowed to perform jam clearing operations on equipment such as, but not limited to, Lincoln Machine Automatic Slab Saw Serial # 89170-D, without receiving training required to perform energy control procedures as an authorized employee.

29 CFR 1910.22(a)(2): The floor of each workroom was not maintained in a clean and, to the extent feasible, in a dry condition. When wet processes are used, drainage was not maintained and, to the extent feasible, dry standing places, such as false floors, platforms, and mats must be provided. (a) Slab Saw Station - On or about August 19, 2019 the employer did not ensure that the work floor of the slab saw station was maintained in a dry condition exposing employees to slip and trip hazards.
