On or about August 21, 2019, at the above addressed jobsite, an employee working on the ground, below roofing activities, was not wearing a hard hat to protect against falling materials and debris.

29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  a.  On or about August 21, 2019, at the above addressed jobsite, employees using pneumatic nail guns did not wear safety glasses, thereby exposing employees to the hazards associated with eye injuries.

On or about August 21, 2019, at the above addressed jobsite, employees performing residential roofing operations, eighteen (18) feet inches above the ground, were not protected from falls, thereby exposing employees to fall hazards.
