29 CFR 1910.119(f)(4): 29 CFR 1910.119(f)(4): The employer did not develop or implement safe work practices for employees and contractors performing high risk maintenance activities on covered processes. Pacific Ethanol Aurora West, LLC - 2103 Harvest Drive - Aurora, Nebraska - The employer is failing to develop and implement appropriate safe lockout/tagout procedures to ensure that process valves were properly locked out during contractor hot work. On or about August 9, 2019, an employee suffered burns from ethanol ignited by contractor welding, when a reboiler low point piping drain valve was left open rather than closed. Lockout procedures did not indicate drain lockouts. This hazard was last documented on or about August 14, 2019.
