29 CFR 1904.29(b)(1): A Log of all recordable work-related injuries and illnesses (OSHA Form 300 or equivalent), was not completed in detail as required by the regulation: Arena Food Service, Inc., Manteno, IL - On or about August 23, 2019, the employer failed to record the following workplace injury in the detail required by the regulation on the OSHA 300 Log for calendar year 2019. a. On or about August 19, 2019, a recordable injury occurred when an employee suffered an electric shock when using the pellet warming machine, resulting in a hospitalization. The information in Column F was not recorded in the detail required, as it did not describe the object that directly injured the employee.
