29 CFR 1910.134(c)(1): A written respiratory protection program that included the provisions in 29 CFR 1910.134(c)(1)(i) - (ix) with worksite specific procedures was not established and implemented for required respirator use: On or about August 21, 2019, employees at B & R Repair, Inc. were required to wear respirators during welding operations in truck and tank trailers. The employer did not establish and implement a written respiratory protection program for these employees. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.146(d)(6): The employer did not provide at least one attendant outside the permit space into which entry was authorized for the duration of entry operations: On or about August 21, 2019, employees at B & R Repair, Inc. were entering permit-required confined spaces to perform repairs on truck and tank trailers. The employer did not provide at least one attendant outside the permit space for the duration of the entry operations. Employees were provided with a fob which controlled a light outside of the permit space in lieu of having an attendant monitoring the space. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(c).
