29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: At a jobsite located at 1330 North Jackson Avenue, Kansas City, Missouri 64123, an employee was exposed to serious struck by and or engulfment hazards. A stairway, ladder, ramp or other means of egress was not located in the trench where the employee was working. The excavation was measured at 7 feet wide, 8 feet 7 inches deep and 11 feet long.
