29 CFR 1910.147(d)(2): Machine or equipment shutdown. The machine or equipment shall be turned off or shut down using the procedures established for the machine or equipment. An orderly shutdown must be utilized to avoid any additional or increased hazard(s) to employees as a result of the equipment stoppage. a. On or about August 6 2019, the employer did not require maintenance employees to deenergize and lockout the Slider Frame Welder Machine during servicing and maintenance activities, which led to an amputation injury.

29 CFR 1904.39(a)(2): Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. a. On or about August 8, 2019, the employer did not report an employee amputation to OSHA within 24 hours. Jeld-Wen was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1904.39(a)(2), which was contained in OSHA inspection number 1380689, citation number 1, item number 1 and was affirmed as a final order on March 15, 2019, with respect to a workplace located at 811 Factory St, Hawkins, WI 54530.
