29 CFR 1926.501(b)(11): "Steep roofs." Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. On or about August 23, 2019, during the installation and removal of roofing material on a church, located at 25200 Lorain Road, North Olmsted, Ohio, the employer did not ensure employees working from an approximately 5/12 pitch roof were protected from falling approximately 21 feet, by utilizing a guardrail system, safety net system and/or personal fall arrest system. STRAIGHTLINE SIDING AND ROOFING, LLC, WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD, 29 CFR 1926.501(b)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 1268638, CITATION NUMBER 01, ITEM NUMBER 002 AND WAS AFFIRMED AS A FINAL ORDER ON OR ABOUT NOVEMBER 14, 2017, WITH RESPECT TO A WORK PLACE LOCATED AT 2906 MAPLECREST AVENUE, PARMA, OHIO.
