29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Valve test pit area: On or prior to 8/22/19, a wall-mounted transformer located below the disconnect for jib crane was not installed in accordance with it's listing and labelling in that it was a single phase, step down type transformer with a primary voltage rating of 600V and secondary voltage rating of 120/240V. The transformer had been wired as a step-up type to output 575V three-phase.

29 CFR 1910.305(e)(2): Switches, circuit breakers and switchboards installed in wet locations were not enclosed in weatherproof enclosures: (a) Valve test pit area: On or prior to 8/22/19, disconnect for jib crane along wall (to right of the clock) was not enclosed in a weather-proof enclosure and had been covered with plastic sheeting. (b) Valve test pit area: On or prior to 8/22/19, disconnect located at the base of the jib crane was not enclosed in weatherproof enclosure and the enclosure was damaged; the disconnect was covered with plastic sheeting to protect it from water.
