The hazard was not guarded to prevent employees from using the walking-working surface until the hazard was corrected or repaired:  a.  On or about August 23, 2019, in the facility, employees were exposed to slip, trip and fall hazards from uneven walking working surfaces throughout storage area which contained uncovered troughs, damaged floors, uneven, crumbling surfaces of various heights.

On or about August 23, 2019, in the bailer room, employees were exposed to fall hazards when the walkway on the bailer operator platform was not guarded to protect employees from a fifty one inch (51") fall.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a. On or about August 23, 2019, in the facility and surrounding property, employees were exposed to contaminated soil which contained but was not limited to polychlorinated biphenyl (PCB), mercury, naphthalene, arsenic, lead and manganese.  In addition, employees were exposed to scrap metals which contained but was not limited to copper, iron and aluminum.  The employer failed to develop and implement a hazard communication program which informed of employees of the hazards present in the workplace.
