29 CFR 1926.451(c)(1): Supported scaffolds with a height to base width (including outrigger supports, if used) ratio of more than four to one (4:1) were not restrained from tipping by guying, tying, bracing, or equivalent means, as set forth in subparagraphs (i) - (iii) of this paragraph: On or about August 21, 2019, the employer failed to ensure that the scaffold that was erected at a commercial job site located at 430 Jefferson Ave., Cleveland OH 44113, was properly secured to the building to prevent it from tipping over. Subcontractor employees who were working on the job site to include on the upper highest level of the building (the roof was already removed) and on the ground were exposed to struck by and caught in between hazards when the erected scaffold collapsed.
