29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.   On or about August 23, 2019 during the installation of roofing material at the job site located at 3203 Broadleaf Way, Brunswick, Oho 44212; the employer did not ensure employees working on the roof of a residential multi-family home were protected from possible danger of eye injury from impact, or from flying objects from the employees installing roofing material with air driven nail guns.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about August 23, 2019, during the installation of roofing material at a job site located at 3203 Broadleaf Way, Brunswick, Ohio 44212; the employer did not ensure employees working on a roof of a multi-family new construction home were protected from a fall of approximately 20.6 feet by utilizing a fall protection system.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about August 23, 2019 at the job site located at 3203 Broadleaf way, Brunswick, Ohio 44212; during the installation of roofing material on a residential roof, the employer did not ensure portable ladders extend 3 feet above the upper landing surface while gaining access and or egress to the working surfaces.  The Miller's Construction & Roofing LLC DBA ALM Roofing & Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard (1926.1053(b)(1) Ladders), which was contained in OSHA inspection number 1392568, citation number 01, item number 01-a and was affirmed as a final order on (7/26/2019), with respect to a workplace located at 1500 Weaver Drive building 6, Granville, Ohio 43023.
