29 CFR 1910.178(l)(3)(iii): The employer did not ensure that each operator had successfully completed training in the requirements of this section: a) Warehouse- on or about April 25, 2019, employees were exposed to struck-by hazards in that the employer did not ensure employees received training that included topics such as but not limited to what to do when the powered industrial truck is to be left unattended. Powered industrial trucks were observed unattended when operators departed for lunch.
