29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: On or about August 20, 2019, Employees retrieving items from the warehouse racks were exposed to a fall hazard because employees were required to step on the racking system platform, which was not protected by a standard railing, safety net systems, or personal fall arrest system.

29 CFR 1910.37(a)(3): Exit routes were not maintained free and unobstructed so that no materials or equipment were placed, either permanently or temporarily, within the exit route: On or about August 20, 2019, Employees working in and around the warehouse area were exposed to fire, smoke inhalation, slips, trips and falls hazards in the event of an emergency because the North Emergency Exit door was obstructed with a washer machine.
