The employer did not ensure that each employee had access to labels on containers of Formaldehyde and to safety data sheets, and is trained in accordance with the requirements of HCS and paragraph (n) of this section: On or about August 27,2019, at 8438 S Dixie Highway, Miami, FL 33143 hair salon, the employer did not include formaldehyde containing products such as Keratin Complex (1.27% formaldehyde) and Premier Brazilian (4.31% formaldehyde) in the company's hazard communication program.
