Observed on or about 08/26/19. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.

29 CFR 1926.451(b)(3)(i): The maximum distance from the face of the work for outrigger scaffold was not kept under 3 inches. a- Building 2 - Kramer Court, Florence NJ: On or about 08/26/19, a company employee was exposed to fall hazards (in excess of 10 feet to the ground surface below) when working and walking from a supported scaffold's outrigger that was greater than 3 inches from the face of the work and the outrigger's walking and working surface was not supplied the required guard rail system(s). In addition this employee was not provided with a personal fall arrest system as an alternative. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1000.00 IN ACCORDANCE WITH 29 CFR 1903.19.
