29 CFR 1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns: (a)3851 Merryweather Trail SW., ground, front and side of dwelling; On or about 09/22/2019 one employee was exposed to struck-by hazards in that he was not required to wear a protective helmet while employees were working 18 feet above him with pneumatic nail guns and hammers.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Dwelling, Roof; On or about 09/22/2019 the employer allowed two employees to be exposed to fall hazards up to 25 feet while they were performing shingling operation from a two story residential dwelling from an 8 in 12 pitch roof without wearing fall protection.
