The employees on the ground cleaning up were not wearing hard hats, exposing employees to struck by hazard, on or about August 22, 2019. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  1059 Glen View Drive York, PA - Employees were using an aluminum extension ladder to access the main roof from the ground at the front of the house, the ladder did not extend three feet above the landing, exposing the employees to injuries from ladder collapse or deflection, on or about August 22, 2019.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.
