Deficiencies noted during the inspection included, but were not limited to, failing to use personal protective equipment such as safety glasses or hardhats, lack of guarding on a belt and pulley on the shingle loader, damaged fall protection equipment being used, a ladder was used that was damaged, twisted and missing a label and failing to use fall protection.
