29 CFR 1910.1096(d)(2): Appropriate personnel monitoring or equipment such as film badges, pocket chambers, pocket dosimeters of film rings were not supplied to or required to be used by employee(s) as described in 29 CFR 1910.1096(d)(2)(i),(d)(2)(ii) and (d)(2)(iii): (a) Lancaster County Pediatric Dentistry - The employer did not provide personnel monitoring devices for the employee(s) operating the x-ray machine, on or about August 27, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: (a) Lancaster County Pediatric Dentistry - Employees used Caviwipes containing; isopropanol, ethanol, 2-butoxyethanol, and didecyldimethylammonium chloride. The employer did not revise the written hazard communication program since the change to Hazard Communication 2012 as evidenced by the numerous references to the MSDS instead of the current SDS, on or about August 27, 2019. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
