29 CFR 1926.452(w)(1): Scaffolds were not plumb, level, and squared:    (a) Jobsite, 2820 Gracy Center Road, Coraopolis, PA:  On or about August 23, 2019 the mobile scaffold being used by employees to paint the structure was not level.  The scaffold was being used on a sloped area and boards were placed under the wheels in an attempt to level it.  The scaffold was raised to a height of approximately 18 feet.  The scaffold began to roll and then tipped over and struck the ground.  Employee 1 sustained a broken pelvis and broken ribs; employee 2 sustained a broken left leg, broken right arm, fractured vertebrae and a concussion.

29 CFR 1926.454(c)(3): The employer failed to retrain employees that showed indications through work performance of not retaining the requisite proficiency:    (a) Facility, 2927 Liberty Way, McKeesport, PA:  On or after August 23, 2019 the employer failed to retrain employees who use mobile scaffolds to perform work to ensure employees retain the required proficiency to operate the equipment safely.  Employees did not adequately level a mobile scaffold which resulted in it tipping over and injuring the employees thereby indicating they had not retained the knowledge and methods to use the equipment in a safe manner.

29 CFR 1904.39(a)(2): The employer did not report within 24-hours a work-related incident resulting in in-patient hospitalization, amputation or the loss of an eye:    (a) Facility, 2927 Liberty Way, McKeesport, PA:  The employer did not report the in-patient hospitalization of two employees within 24 hours of the work related incident.  Employees were injured at approximately 12:00 pm on August 23, 2019 and the employer did not report the hospitalization to the Pittsburgh OSHA office until August 26, 2019.
