29 CFR 1926.453(a)(2): Aerial lifts were "field modified" for uses other than those intended by the manufacturer, the modification had not been certified in writing by the manufacturer or by any other equivalent entity. Modification did not conform with all applicable provisions of ANSI A92.2-1969: a) On or about August 22, 2019, at the jobsite, the aerial lift Genie S-40 (HR2 rental, number 400-330) was used to lift roof joists by attaching with a chain to the platform exposing employees to struck-by hazards.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a) On or about August 22, 2019, at the jobsite, fall protection was not used by employees installing roof joists while standing on tope of the 11 feet 8 inches high walls and base of joists, exposing them to a fall hazard to the concrete floor of the new commercial building.
