29 CFR 1910.178(p)(1): Powered industrial truck(s) found to be in need of repair, defective, or in any way unsafe had not been taken out of service until restored to safe operating condition. On or about August 28, 2019, employees were exposed to struck by hazards in the warehouse while working where Powered Industrial Trucks (PITs) were used and were not removed from service after defects such as inoperable parking brakes, lights, back up alarms, and propane tank straps were reported.
