29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, the following:    (A) A specific statement of the intended use of the procedure;   (B) Specific procedural steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy;   (C) Specific procedural steps for the placement, removal and transfer of lockout devices or tagout devices and the responsibility for them; and  (D) Specific requirements for testing a machine or equipment to determine and verify the effectiveness of lockout devices, tagout devices, and other energy measures.     a)On or about 8/28/2019 the employer did not have written procedures to control the hazardous energy when employees are performing maintenance or repairs, such as but not limited to; changing of the blades on machines or equipment such as but not limited to the WF Wells Band-saw and  HYD MEC H10A.

29 CFR 1910.147(c)(6)(i): The employer shall conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.  a) On or about 8/28/2019 the employer did not conduct a periodic inspection of the energy control procedures to ensure the procedures and requirements were being followed, such as but not limit to; maintenance or repairs such as changing the saw blades for the WF Wells Band-saw and HYD MEC H10A.

29 CFR 1910.147(d): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence:    a) Employees performing servicing and maintenance on the WF Wells bandsaw and HYD MEC H10A bandsaw were not required to:    1) Turn off and shut down the power to the WF Wells bandsaw and HYD MEC H10A bandsaw.   2) Physically locate and operate energy isolating devices in such a manner as to isolate machine or equipment from the energy source.   3) Affix lockout or tag-out devices to energy isolating electrical device(s); and   4) Verify that isolating and de-energization of the machine or equipment of the machine or equipment have been accomplished.    a)On or about 8/28/2019 employees in the process of changing the blades on the WF Wells bandsaw and HYD MEC H10A bandsaw in the facility were not required to lockout the energy sources.
