29 CFR 1910.219(f)(3): Sprocket wheels and chains which were 7 feet or less above floors or platforms were not enclosed: On or about August 23, 2019, and at times prior there to, employees were performing maintenance work on a TDU machine that had an unenclosed chain and sprocket mechanism. The employee was exposed to amputation and caught-in between hazards.
