29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:     The employer is failing to ensure that all employees exposed to fall hazards while working on a residential roof are utilizing fall protection.  This most recently occurred on August 28, 2019 on the jobsite located at 10212 Krug Ave., Omaha, NE 68124.  The owner and 4 employees were observed installing shingles on an existing single-story house without any form fall protection.  Height from the ground to the edge of the roof is approximately 8 feet and the roof has a 4 in 12 pitch.
